Facilitator Certification Registration Form

Please read this page carefully. Your continued Certification is dependent on your abiding by this agreement at all times.

Upon execution, this Registration Form ("Agreement") shall become binding upon the undersigned Certification candidate (referred to herein as "you"). AchieveGlobal is committed to helping each customer meet its goals through the application of the skills presented in the training modules and programs it has purchased. In keeping with that commitment, this Agreement defines the terms and conditions governing your Certification. Your continued Certification is dependent on your abiding by this Agreement at all times.

Upon successful completion of all Certification requirements for the programs and modules listed below (the "Programs"), you will be Certified to use the Programs to train only those persons for whom the AchieveGlobal customer listed on this Agreement as your current employer has purchased licensed Programs and only in accordance with the AchieveGlobal training policies, including, without limitation, permitted class sizes. If granted, your Certification will not give you the right (as an independent contractor, consultant or otherwise) to train any individuals for whom your current employer has not purchased Programs. The only exception to the foregoing rule is that you may contract to provide Program training for authorized AchieveNET distributors at their request.

All AchieveGlobal materials (e.g., facilitator guides, leader guides, instructor guides, videos, wall charts, workbooks, activity cards, and program books) may be used only within your current employer's organization, and only certified trainers may deliver certain components of Program training (e.g. classroom delivery of core Program sessions).

Concepts or models from the Programs cannot be copied onto posters, mugs, T-shirts, Internet sites or intranet sites etc., to be used in and around your current employer without securing a limited rights license to reproduce such items as part of your current employer's Master Agreement with AchieveGlobal. Your AchieveGlobal sales representative can help you obtain these rights under your current employer's agreement.

Materials are designed to be used exactly as produced by AchieveGlobal. All Programs and associated training must be delivered in their full and unaltered state. The materials may not be photocopied, reproduced or modified without AchieveGlobal's prior written consent.

Each participant who goes through Program training must have his or her own individual set of participant materials, which may not be reused by subsequent participants. Materials (including videos and wall charts) may be used only as part of the total process, and you agree to use the videos only with the corresponding training program or module.

You may print, photocopy and/or distribute only those portions of the material labeled, "This page may be reproduced under license."

If granted, your Certification will permit you to train participants using the Programs only in the language in which you are certified. If you are interested in delivering training in other languages, please contact your local office or sales representative.

All facilitator materials are, and shall remain, the property of your current employer. You may not retain possession of them for yourself or take them with you if you leave your current employer. If your new employer wishes you to deliver AchieveGlobal training, it will be necessary for the new employer to enter into a contract relationship with AchieveGlobal and to purchase for you any necessary facilitator materials. Although you might not be required to repeat the entire certification process, AchieveGlobal does require you to update your contact information in AchieveGlobal's Facilitator Database and submit a new Certification Agreement in order to ensure your continued Certified status at your new employer. After you deliver such information to AchieveGlobal, your new employer will be deemed your "current employer" for purposes of this Agreement.

If granted, your Certification will give you the right to deliver training only in connection with those Programs listed below; it does not give you the right to deliver training in connection with any other programs or modules offered by AchieveGlobal. Should a Program undergo a significant revision (as determined by AchieveGlobal), AchieveGlobal may require that you complete additional certification in order to deliver training in connection with the revised Program.

AchieveGlobal reserves the following rights in regard to your Certification: (i) to grant Certification upon your completion of Certification training; (ii) to not grant Certification upon your completion of Certification training; (iii) to conditionally grant Certification upon your completion of Certification training, meaning that additional development and review may be required before you become unconditionally Certified; (iv) to rescind your Certification if AchieveGlobal reasonably determines that you are not training in accordance with AchieveGlobal's performance and certification standards; and (v) to rescind your Certification if you at any time fail or refuse to take any actions reasonably requested by AchieveGlobal to facilitate communications with AchieveGlobal and to keep current your knowledge regarding AchieveGlobal's products, policies and procedures.
